Culturally Effective Healthcare Development Project at Laguna Honda

The Culturally Effective Healthcare Development Project aims to improve cultural and linguistic competency throughout the Laguna Honda Hospital and Rehabilitation Center and thereby reduce disparities in patient/resident service provision.

The hospital provides skilled nursing and rehabilitation services to 780 residents. It is among the largest nursing facilities in the country and an integral part of the San Francisco Department of Public Health’s continuum of care, which offers skilled nursing, assisted living, and independent living with wrap-around support services to a safety-net population. Laguna Honda’s model of care is resident-centered and offers an interdisciplinary approach that combines professional services from a range of competencies, including nursing, medicine, rehabilitation, activity therapy, nutrition, and social work.

This project aims to provide culturally competent care and to achieve a level of functionality that will allow residents to transition to community settings.
