Arthur H. Coleman Community Health Foundation

Founded:
September 2006. By Pat Coleman, M.J. and Marilyn Metz, M.D.

History:
Founded to provide preventive health and chronic disease management services to the community, in the memory of the late Dr. Arthur H. Coleman, family physician. Dr. Coleman faithfully served patients in Bayview Hunters Point and San Francisco for 54 years, delivering compassionate, culturally competent, high-quality primary medical care.

Mission Statement:
It is the mission of the Arthur H. Coleman Community Health Foundation to help eliminate health disparities in the Bayview Hunters Point neighborhood of San Francisco by providing free programs in health education, healthful lifestyle support, and chronic disease prevention and management to all community members in need.
